You are right, the AP in question is from Cisco. So let me summarize:

WEP (static or dynamic): better make no assumption on broadcast/unicast
key indices. 
TKIP and CCMP: pairwise key index 0, group keys indices 1..3

In that case, my problem is with the ieee80211 stack used by ipw2200,
the wireless extensions interface in version 1.1.6 does not allow me to
set unicast wep keys at index != 0. I'll send a patch to the
ipw2100-devel list.
