one radio program in a series causes ffmpeg to throw an error - is this unusual?

Looks like a duff file. Newer versions of ffmpeg will flag the error but
keep re-muxing. Since you appear to have some distro in the Ubuntu
family, just use the default avconv (should have been installed with PPA
build of get_iplayer) instead of an ancient version of ffmpeg. If you're
anti-libav or wedded to your old ffmpeg for some reason, try
--modes=hlsbest and see if that makes a difference.
