Adding reserve=0xff490000,0x00aff000 fixed the crash, thanks.

Sure.  I can't easily get a dmesg of the crashing case, but here's one
for the working case, where the card is inserted after udev is up.  I've
tried pci=routeirq and pci=assign-busses in case either was relevant, as
suggested by dmesg, but they didn't change anything.
