Memory include fails during first run of cardmgr

I switched from pcmcia-cs to pcmciautils+hotplug. Now, problem 1) obviously 
disappears since cardmgr is no longer required.

However, problem 2) remains, i.e. only second "card insert event" is noted 
from the system.

Any idea what could be the reason for that?
