Management frames in 0.0.4 and later

Hi,

over a month ago, I have reported problems with hostap-0.0.4 and later 
versions in a production network that the hostAP was not able to send 
management frames properly to stations trying to associate. Jouni has 
suggested to dump some diag output (hostap_diag -a) and to try to reset the 
card. I am really sorry it took so long, but we were busy with other things 
here and once I gather all the info, I had to leave for a business trip 
before I had a chance to write this little summary.

OK, attached are the 3 hostap_debug outputs - the good one from 0.0.3, the 
bad one from 0.0.4 and the good one from 0.0.4 after iwpriv wlan2 reset 0 
(reset 2 did not help, but reset 0 DID help).

Executing reset 0 solves the problem, but I am sure there is a way to fix it 
in the driver initialization code.

Just looking at diffs of the hostap_diag data, I would suspect CR8, but most 
of the CRs do not have any meaning to me, as I am not that deep into the 
PRISM internals.
