As I keep some older versions of the main script in my
installation folder (for experimentation & debugging purposes),
I conducted some further experiments with your sample pid=b048s4tn.

With my oldest version
get_iplayer v2.90-45-g16b0c40 of 2014-12-06
(http://git.infradead.org/get_iplayer.git/blob_plain/16b0c40f1ca65c0ccfec23e72928d17f8b5ec862:/get_iplayer),
I get:

A. NO --playlist-metadata
Plants_From_Roots_to_Riches_-_1._A_Rose_by_Any_Other_Name_b048s4tn_default
B. --playlist-metadata
Plants_-_From_Roots_to_Riches_A_Rose_by_Any_Other_Name_b048s4tn_default

So, already a small difference between the auto-generated file names, but no 
info missing.

With my slightly newer copy
get_iplayer v2.90-58-gd1caff0 of 2014-12-12
(http://git.infradead.org/get_iplayer.git/blob_plain/d1caff0e19139eba4f4e6cac826a6ebf7061e295:/get_iplayer),
I get:

A. NO --playlist-metadata
Plants_From_Roots_to_Riches_-_1._A_Rose_by_Any_Other_Name_b048s4tn_default
B. --playlist-metadata
Plants_-__b048s4tn_default

Tag v2.91 exhibits the same behaviour and this, as you state,
is matched by the Jan 3rd dev version of the script.

I would look in the commits between 16b0c40 & d1caff0
to find the change of code that introduced this "bug"...
