pointers in the libertas USB driver

In order to wrap my mind around most of the pointers I made an 
illustration.

The attached picture contains:

* structure names (at the top of the boxes)
* most common variable names (above the boxes)
* pointers to other mentioned structures
  (inside the boxes)
* what points to what (arrows)
* where the pointer is set (the vertical text at the
  arrows)
