make debug output selectable

In my cleanup tree is a (huge) patch that allows one to specify
which code parts should emit debug output and which not.

For example, to get debug output on Scan & Assoc, one can do

   modprobe usb8xxx debug=0x180

See defs.h for the possible bitmask. If accepted, a later patch will
update the README.

This is shaped similar to the debug code in the ipwXXXX drivers.
