How to detect a "misflashed" card?

Did you change the card to managed mode? The default mode for Host AP
driver is Master/AP and if you are trying to associate with an AP, you
will need to set the card in managed mode (iwconfig wlan0 mode managed).
Normal behavior of the Prism2 cards is to blink the led in Host AP mode.
