refresh problems

I am having some problems with refreshing the tv cache and have boiled
it down to this:

I start with no tv.cache file at all, the next refresh command
successfully updates and my tv.cache file is created with 365 KB

A subsequent refresh then updates, find 0 mathcing and my tv.cache
file drops to 1 KB

What's going wrong?
