FYI, I don't think openconnect_setup_tun_fd() works anymore after the
tunnel setup reordering change.  How hard would it be to support
existing library users who try to set up the tunnel before entering
the mainloop?

I think we may also need to add a new callback to tell the app when
the MTU is known and we're ready to set up the tunnel interface.
