Can't connect to Juniper VPN because: "Unknown form ID 'frmSelectRoles'"

Alright so I manually merged in your original cleaned up patch after
reverting the committed changes and it worked swimmingly, so you
definitely lost something in the conversion. Also, is there any way
you can number the options for the available roles?
