[RFC][MTD] Fix incorrect interface code for x16/x32 chips

According to "Common Flash Memory Interface Publication 100" dated December 1,
2001, the interface code for x16/x32 chips is 0x0005, and not 0x0004 used so
far.

Caveats:

- Is this a bug? The 0x0005 code is found in a fairly old document, so perhaps
  things have changed since then and x16/x32 interface has now code 0x0004? I
  have been unable to locate more recent CFI specs that would explain it, so
  I'd like to hear other opinions on this.

- I don't have access to a x16/x32 chip, so I'd appreciate somebody testing
  the patch on such a device in x16 mode.
