Merging the coverity_scan branch

Howdy,

It is time to merge the coverity_scan branch into master.
The number of changes is huge.

coverity_scan is the branch where I've been cleaning up the issues from Coverity Scan, Splint, etc.
and also where I ran GNU indent to enforce the coding style standard.

If you are working on a big patch from master and don't want me to merge yet, please speak up ASAP.
