KMS update, and afk for a week

Major progress on KMS support today, in the vc4-kms-pv branch.  Most of
the body of the modeset functions in encoder and CRTC are filled in now.
I'm optimistic about having it ready for 4.3, assuming we can get
firmware and dt-clocks in.

As a note to those providing review on the patches I have outstanding,
I'll be gone for the next week.  If anyone wants to pick up doing the
polishing on them during that time, it would be great.
