[PATCH] PCMCIA: Delete obsolete PCMCIA_IOCTL feature.

  Delete all code and configuration info related to the obsolete
PCMCIA_IOCTL config setting.

scheduled for removal back in 2005.  compile-tested with "make
allyesconfig" on x86.
