[PATCH] nvme: fix command effects log lifetime for multipath heads

Hi all,

A gentle ping on this patch. It fixes a KASAN-reported use-after-free
in the multipath namespace head ioctl path by keeping the command
effects log pointer with each controller path.

A blktests regression test was also posted as nvme/070:
