[PATCHv2] nvme: Delete created IO queues on reset

Thu Oct 6 10:15:12 PDT 2016
The driver was decrementing the online_queues prior to to its attempt
to delete those IO queues, so the driver ended up not requesting the
controller delete any. This patch uses the saves the online_queues prior
to suspending them, and adds that parameter for deleting io queues.
Fixes: c21377f8 ("nvme: Suspend all queues before deletion")

v1 -> v2:
Instead of using queue_count, this takes a snapshot of the online
queues and makes that a parameter.
