write thread not run in readonly mode.
fixed bug in readpage() when page found in outgoing write queue.
added kiobuf fix for pre 2.4.4.
doesnt make copies of erase pages into outgoing write queue, just keeps one
  erase page around for ever.
name now set in mtd_info struct according to device name (eg /dev/hda1)
