It could be nice to have all patches properly formatted and updated
(refreshed) all the time, but it's sometimes easy to miss that.

I don't care about having commits *that* cleanly separated, as long as
refreshes are trivial, I'm OK with that. It may be you're spending
more time on this trivial issue than it's worth ;)
