What's the meaning of wireless event "AP: 00:00:00:00:00:00"?

 It make the wireless connection drops, and the log like below, so
what's the possible reason? Thanks!

Setting scan request: 0 sec 100000 usec
Added BSSID 40:f4:ec:7e:17:de into blacklist
CTRL-EVENT-DISCONNECTED - Disconnect event - remove keys
