Indeed, it did not increase.  The replay counter was 1; it needed to be
larger than 3.

The replay counter is supposed to be zeroed on every (re)association on
both ends.  If that's what happened here (and this wasn't just a re-key
interval expiring), then it's likely that wpa_supplicant just wasn't
notified that the association dropped.  On Windows, under wpa_supplicant
0.4.x, this can happen if ndis_events.exe isn't running, but I don't
know what can cause it with the wireless extensions backend.

Unless this wasn't a reassociation -- if it was just a re-key, then it
may be a bug in your AP.  (I think.)  Possibly try newer firmware, if
there is any?
