This line is not correct. For this card you need pm010102.hex from

What is your installation method? Are you using the hostap-drivers ebuild, an 
external build of the hostap-drivers package at, or 
patching your kernel?

It seems the primary firmware is never downloaded. This might be because you 
have the wrong file in hostap_fw_load.
