my hardware d-link DWL660 PCMCIA (H/W:A1 F/W:4.04) seems not to be a 
PRISM2.5 ?
With the Orinoco driver a Prism2 Card is displayed and it works.
The card ID:     manf ID 0x156, 0x0002 seems to be the id of a Prism2 Card.
Hostap_diag wlan0     displays:
NICID: id=0x0001 v5.0.4 (unknown)
PRIID: id=0x0015 v4.4.1
STAID: id=0x001f v8.10.1 (station firmware)

th host ap driver works only in ad-hoc mode.

I  had seen an older mail to the same problem in  your list. What for a 
Chipset has this Card and does the card work with hostap in master mode?
