I have the same card, and had the same problem. Basically if you
didn't do a clean module unload, and you reboot, the system will crash
the next time you load the module. Solution is do a cold reboot
(shutdown and then start up again).

I also found out that sometimes (I am not sure if it's always yet), if
you unload modules and load them again, that will bring down the
system as well. Also last time (the onlyt time) I tried to use kismet
with it, and that also results in a crash.

If you have another card, or another revision of the same card, I
highly recommend you use that instead to save yourself some time.
