get_iplayer v2.78 release

The Akamai downloads are fixed properly and the code is cleaned up a
bit. It'll now use rtmpdump by preference instead of flvstreamer, and
will use the '--swfVfy' argument instead of '--swfUrl' if it'll be
understood. We've reinstated the self-update facility, and the Windows
installer works again. New 'flashlow' modes are supported.
